Referral Instructions:

Talk to the family. Make sure they would like to use Child Service Planning as a resource. Invite them to attend, if they are able. Meetings are currently held via zoom. 

Have caregiver complete the Release of Information, click here

Complete the Referral Form. Click here to complete and submit. 

What is next: Once the release is received and the referral is complete, the family will be scheduled.  Agendas will be emailed out with the date/time of the meeting. All meetings will take place on Wednesdays from either 9-10am or 10-11 am
