Personal Activity Reports (PAR) are completed by Earlywood employees who are supporting students who are educated in private schools or home school settings. 

 

PARs must be completed monthly by the last day of the calendar month that services were provided. Multiple students can be listed on the same report. The report must be signed prior to being submitted and sent electronically to the business office. 

 

PAR must be completed for direct services and case conferences. 

 

Homeschool: Students not enrolled in public school or another private school.  They are educated at home.  

Walk-in: students that qualify to attend public education, but parents elect to send them to private school that we do not support.  Receive OT, SLP, etc.. services on a walk-in basis in the public schools.  

Private: students that are enrolled at St. Rose or OLG.
